Patient Access to Medical Records

Patients may access their health information records held by Mater in accordance with the Commonwealth Privacy Amendment (Private Sector) Act 2000.

You have the right to access and/or collect personal information that we hold about you subject to the Privact Act 1988.

If you wish to access your personal information/medical record, you should make your request in writing to the Health Information Manager. Please complete the attached form, returning this together with a copy of photo identification in person or via email.

Please be aware that there may be a fee involved when accessing your records. For further information regarding the fees, please click here.

Transfer of Patient Information

If you require your Medical Record to be transferered to another Facility or Doctor, your consent is required. To provide consent, please fill in the attached  form.
